Clients are responsible for providing complete and accurate information about any known allergies, ingredient sensitivities, medication sensitivities, previous reactions, health conditions, medications, recent procedures, or other factors that could affect treatment.

This information must be disclosed through the appropriate intake forms before the first treatment.

Before each subsequent appointment, clients are responsible for letting Composition know about any new or changed information that could affect treatment, including new allergies or sensitivities, medication changes, recent procedures, new health conditions, pregnancy, or previous reactions.

Composition relies on the information you provide when selecting products and determining whether a service is appropriate.

A service may be modified, postponed, or declined if an allergy, sensitivity, contraindication, or other safety concern is identified.

While reasonable precautions are taken, it is not possible to guarantee that an allergic or sensitivity reaction will not occur.

If you experience an unexpected reaction following a treatment, discontinue use of any related home-care product when applicable and contact Composition at contact@compositionesthetics.com.

Seek appropriate medical care for symptoms that require medical evaluation or treatment.
