Composition LLC, doing business as Composition ("Composition," "we," "us," or "our"), respects the privacy of our clients and website visitors.
This Privacy Policy explains the types of personal information we collect, how we use it, and when it may be shared when you visit compositionesthetics.com, schedule an appointment, receive services, purchase products, or communicate with Composition.
Information We Collect
Website Information
Our public website is hosted by Squarespace.
When you visit the website, Squarespace may process information about your browser, device, network, IP address, pages viewed, website activity, and related technical information needed to operate the site and provide analytics.
If you submit a general website form, we receive the information you choose to provide.
Please do not submit sensitive health or treatment information through general website contact forms. Health and treatment information should be provided through the appropriate Acuity intake form or directly to Composition through an appropriate secure method.
Booking and Contact Information
Composition uses Acuity Scheduling for appointment booking and client management.
When you schedule an appointment, we may collect information including:
name;
email address;
telephone number;
appointment date and time;
service booked;
information submitted through intake or consent forms;
files you choose to upload;
appointment history; and
other information you provide during the booking process.
Health and Treatment Information
To provide esthetic services safely and maintain appropriate client records, Composition may collect information including:
skincare concerns and goals;
allergies and sensitivities;
relevant medications;
relevant health information;
previous treatments or procedures;
contraindications and treatment considerations;
services performed;
treatment notes;
treatment response or complications; and
recommendations or aftercare information.
Composition uses HIPAA-enabled Acuity Scheduling to collect and maintain sensitive client information used for appointment intake and esthetic treatment records.
Sensitive health and treatment information should be submitted through the forms provided through Acuity or shared directly with Composition through an appropriate secure method.
Please do not send health information, medications, allergies, treatment history, or other sensitive client information through general Squarespace contact forms, social media messages, or other general website communications.
Acuity's HIPAA-related safeguards apply to information maintained within our HIPAA-enabled Acuity account. Composition does not use standard Squarespace website forms to collect or maintain sensitive health or treatment information.
Photographs
When applicable, photographs may be taken or maintained as part of treatment documentation or progress tracking.
Treatment documentation does not authorize Composition to use an identifiable photograph for advertising, social media, or other promotional purposes.
Any promotional use of identifiable client photographs is handled through separate authorization.
Payment Information
Payment information provided when booking or purchasing is handled through the payment-processing services connected to our Squarespace or Acuity systems.
Composition may maintain information about transactions, charges, refunds, and purchase history as necessary to operate the business and maintain financial records.
How We Use Information
We may use personal information to:
schedule and manage appointments;
provide appointment confirmations and reminders;
allow clients to manage appointments;
provide esthetic services;
review allergies, contraindications, and treatment considerations;
maintain required client and treatment records;
document treatment history and response;
provide recommendations and aftercare;
process payments;
administer cancellation and no-show policies;
respond to client questions;
maintain accounting, tax, insurance, and business records;
comply with professional licensing and legal requirements;
maintain the security of our business and systems; and
understand and improve website use.
If you separately choose to receive promotional communications, your contact information may also be used for that purpose.
Oregon Client Records
Composition maintains client records in accordance with applicable Oregon esthetics requirements.
Required esthetics client records are retained for at least three years.
Some information may be retained longer when reasonably necessary for an ongoing client relationship or applicable legal, accounting, tax, insurance, or dispute-related requirements.
Squarespace and Acuity
Squarespace hosts Composition's public website.
Acuity Scheduling is used for appointment scheduling, intake and consent forms, client information, appointment communications, and related client-management functions.
Composition has enabled Acuity's HIPAA features and uses Acuity for sensitive health and treatment information collected electronically as part of the client relationship.
Standard Squarespace website forms are not used to collect or maintain sensitive health or treatment records.
Cookies and Analytics
Squarespace uses cookies and similar technologies necessary to operate the website.
Depending on our website settings and your cookie preferences, analytics and performance cookies may also be used to understand website traffic and activity.
Where a cookie preference tool is provided, you may use it to manage applicable non-essential cookies.
Appointment Communications
Composition may send appointment confirmations, reminders, rescheduling notifications, cancellation confirmations, receipts, and other service-related communications through Acuity.
Appointment communications are intended for scheduling and service-related purposes. Sensitive intake responses and treatment records are maintained within the appropriate client-record system rather than included in general appointment communications.
If you separately opt in to promotional communications, you may unsubscribe using the method provided with those messages.
Opting out of promotional communications does not stop communications necessary to manage an appointment, transaction, or existing client relationship.
When We Share Information
Personal information may be processed or made available to service providers that help Composition operate, including:
Squarespace and Acuity Scheduling;
payment processors;
technology or communication providers used in connection with our services;
accountants, attorneys, insurers, or other professional advisers when reasonably necessary; and
government or regulatory authorities when required by law.
We may also disclose information when reasonably necessary to respond to legal process, address fraud or security issues, protect legal rights, or protect someone's safety.
Data Security
Composition uses reasonable administrative, technical, and physical safeguards appropriate to the nature of the information we maintain.
Sensitive client information collected electronically for treatment and intake purposes is maintained through our HIPAA-enabled Acuity account rather than through general Squarespace website forms.
No electronic system can be guaranteed to be completely secure.
Your Information
You may contact Composition to:
ask about personal information we maintain about you;
request correction of inaccurate information;
request a copy of certain information; or
request deletion when Composition is not legally or professionally required to retain the information.
We may need to verify your identity before completing a request.
Certain client records cannot be deleted while Composition is required to retain them under Oregon law or another applicable legal requirement.
Changes to This Privacy Policy
We may update this Privacy Policy when our technology, services, business practices, or legal obligations change.
The effective date at the top of this page identifies the current version.
